Free Vaccines for Life Enrollment Form
Additional Information
Free Vaccines for Life
Once a pet ( dog or cat, puppy or kitten) is "enrolled," any core vaccines (see list below) it receives are free for its lifetime.
There is a one time "enrollment" or "membership" fee of $129 per pet for new and existing clients.
As long as that pet is examined by the doctor once a year, enrollment/membership will remain current.
Depending on what you currently pay for vaccines, the age of your pet at enrollment, and its vaccine schedule, the plan will provide substantial savings.
Vaccines (anywhere from $20-50 each at OUR hospital - potentially much higher at other hospitals) are free with a paid exam and include:
- Dogs - DHPP (Distemper/Parvo ), Leptospirosis, Bordetella, Rabies
- Cats - FVRCP (Rhinotracheitis/Calici/Panleuk), Feline Leukemia, Rabies
Special order vaccines or vaccines not mentioned above (those NOT routinely recommended such as the Canine Influenza) are NOT included. The Lyme vaccine will be discounted at 50%.
Other services and products received or purchased at the time of the vet exam/visit are NOT free and are at an additional cost to the doctor exam fee. These additional costs include services and products such as heartworm tests, fecal examinations, de-worming, flea treatments, heartworm prevention, and feline leukemia tests.
While a vet exam is only required once a year to keep enrollment current, we recommend exams for puppies and kittens under 4 months of age every 2-3 weeks. For pets 8 years and over, we recommend exams every 6 months.

*What happens if I do not make it in to see the doctor before my year passes?
If you do not come in to see the doctor within one month of its due date then your policy becomes void. You will have to purchase any vaccines from that point on or if the plan is still being offered you may re-enroll.
*What about Lyme and Flu vaccines?
Lyme Disease is becoming more prevalent in our area. It is even being diagnosed in dogs that aren't in high risk lifestyles, (i.e. hunting, spending time outside in wooded areas). During peak tick season (Summer months) we can see an upwards of 4-5 positive cases of Lyme per week with or without showing any clinical signs. The Lyme vaccine is NOT included in this package, but members of the Free Vaccines for Life program will receive a 50% discount on vaccination if they choose to receive this vaccine.
Canine Influenza has a very high prevalence in Indiana. There is no need for direct contact as the virus can survive on surfaces for up to 48 hours. The virus can even be brought into the home on people's clothes and shoes. Clinical signs include fever, coughing, sneezing, runny nose, watery eyes, etc. and if left untreated can develop into secondary infections such as pneumonia. The Canine Influenza vaccine is not discounted with the Free Vaccines for Life program at this time.
